Hospice Care
SunCare Hospice provides expert, in-home medical care for those who are no longer seeking a cure for their advanced illness. The focus of Hospice is on medical comfort, emotional wellbeing and social and spiritual wellbeing of each person and their caregivers.
SunCare Hospice Services are provided by a hospice care team wherever the patient calls home. We offer 4 levels of care depending on the needs of each patient.
- Routine Home Care is provided wherever the patient calls home. It can be a personal home, nursing home or assisted living facility.
- General In- Patient Care (GIP) – is provided short-term around-the-clock care provided in the hospital or other contracted facility.
- Continuous Care– When the patient needs GIP care but would rather stay at home, Continuous care can be provided. This is short-term care provided in your home by your care team in a time of crisis. The goal is to manage symptoms.
- Respite Care – Short-term care, up to 5 days is provided at an extended care facility or contracted hospital. This allows the caregiver to rest, recharge or even take a short trip.
General Inpatient Care and Respite Care will also be provided in our SunCare Center of Springfield, which is slated to open early 2025.
